In this benefit you can choose from a number of life insurance options for your Spouse and complete the beneficiary choices. The cost of the coverage may be based on a single rate or a rate calculated by using your Spouse's age, sex and smoking status. Some options may require your Spouse to provide evidence of their insurability (EOI) as indicated by the heading [Evidence Required]. A tab will appear if EOI is required called [EOI Pending]. At the end of the enrollment after you have hit the "Confirm" button you will be provided the required EOI forms for printing. The form is required by the insurance company to determine whether your Spouse satisfies their health requirements. While awaiting the approval the site will automatically enroll your Spouse in an option that does not require EOI. Once approval has been established your plan will automatically be brought up to date and the coverage will be effective from that time forward. You will receive notice in writing from [Carrier].
