In this benefit you can choose from a number of Critical Illness insurance options. The CI insurance pays a tax free benefit if you are to survive a critical illness as defined in the Plan Details (see INFO icon at the top of the page). The cost of the coverage may be based on a single rate or a rate calculated by using your age, sex and smoking status. Some options may require you to provide evidence of their insurability (EOI) as indicated by the heading [Evidence Required]. A tab will appear if EOI is required called [EOI Pending]. At the end of the enrollment after you have hit the "Confirm" button you will be provided the required EOI forms for printing. The form is required by the insurance company to determine whether you satisfy their health requirements. While awaiting the approval the site will automatically enroll you in an option that does not require EOI. Once approval has been established your plan will automatically be brought up to date and the coverage will be effective from that time forward. You will receive notice in writing from [Carrier].
