This page allows you to choose which enrollment option you want to perform and to obtain forms and other information that might be helpful during the enrollment.
Click on the option that describes what you would like to do and follow the instructions on the pages. You can cancel whatever you are doing during the session and begin again at a later date. When you are satisfied with the selections or changes you have made you must confirm at the end of the process otherwise your changes will not be saved.
Life Events are changes to your circumstances i.e., getting married, having a child, losing a spouses coverage. You can perform a Life Event at any time during the year. The date of the Life Event can be backdated up to 30 days. However, you cannot future date a Life Event.
