Select Child Life Insurance Coverage

This page provides you with the opportunity to choose various life insurance options for your children. The selection of some options may require evidence of insurability (EOI). If evidence is required you will see a pop-up menu offering the appropriate form for you to download and print out. The form is required by the insurance company to determine whether your children satisfy the health requirements. While awaiting the approval your child will be automatically enrolled in an option that does not require EOI. Once the approval has been received your plan will be brought up to date and the coverage will be effective from that time forward.
