Maximum without evidence of insurability: $40,000
Maximum with proof of insurability: $250,000
Termination of benefit: termination of employment, retirement or age 65 of participant.
At the end of the enrollment after you have click on the «Confirm », you will be provided the required evidence of insurability forms. You will need to complete, sign and return these forms to the address indicated in the instructions.
