Maximum with proof of insurability: $250,000
Termination of benefit: termination of employment, retirement, age 65 of participant or age 65 of spouse
At the end of the enrollment after you click on «Confirm », you will be provided the required evidence of insurability forms. You will need to complete, sign and return these forms to the address indicated in the instructions.
