Termination of benefit: Your 65th birthday or retirement if earlier
Beneficiary: Clicking on the « Confirm » button at the end of your enrollment is considered by Blue Cross as an electronic signature. You do not have to return a signed form. However, a beneficiary designation form will be available at the end of your enrollment for your records.
