Amount of insurance for the spouse: Units of $5,000
Maximum without evidence of insurability: For an initial enrollment, the maximum without evidence of insurability is $10,000. For a life event or annual re-enrollment period, an evidence of insurability will be needed for any increase.
Maximum with evidence of insurability: $200,000
Termination of benefit: Your retirement or your spouse’s 70th birthday, whichever comes first
